Screen 1 of 4, What this is
The application takes several minutes and helps me decide whether a longer conversation makes sense. If anything stands out or warrants more discussion, we can cover it on the next free 30-minute call.
Four screens. This one, what to read before you apply, the form itself, and what happens after you send it.
This offer supports a microdosing protocol designed to meet your individual needs. What follows needs your consideration before you can apply. Read it and think about each section. Nothing from this section is asked about on the application or stored anywhere.
If you're having thoughts of suicide, please get support before you read any further.
In the US, call or text 988. Anywhere else, your local emergency number. I'll still be here afterward.
This work is for adults, 18 and over.
You can still apply. Before we start I need to know your doctor has cleared you for this, not only that you raised it with them. What the two of you discuss stays with them.
Pregnant, breastfeeding or trying to conceive: not now. Apply when that changes.
Not a refusal, and not the same as an SSRI either. An MAOI removes one of the routes your body uses to clear a psychedelic, so the effect runs longer and stronger than the dose suggests, and it is the combination where serotonin toxicity is a real concern rather than a theoretical one. If you take one, I need to know before anything starts, and your prescriber comes first.

Lithium taken with psychedelics carries a severe, documented risk of triggering generalized seizures. If you are taking lithium, I do not feel comfortable supporting the protocol.
Seizures in 47% of 62 reported lithium and psychedelic combinations, and in none of 34 on lamotrigine. Nayak and others, Pharmacopsychiatry 2021. PubMed 34348413
If any of these is in your history, whether or not it is active now, I'm the wrong person for this work, and I'd rather say so now than after you've filled in a form. Skip it and email me at the address on the site. I'll point you toward people who work with this properly.
Research safety guidelines screen wider than I do. They exclude a family history of psychosis or bipolar disorder as well as a personal one. I don't treat a relative's diagnosis as a bar by itself, and I do want to hear about it on the call.

The medication list describes where things stand right now, and right now changes. A prescription ends. If that happens, apply then, or write to me and ask.
These are commonly screened in this space. It isn't an exhaustive list, and I'm not a clinician.
Just enough to know whether it's worth a conversation.
Questions marked * need an answer.
A job title is fine. Nothing here rules you out on it.
10¼+ hours of calls, a weekly slot, and a short daily log that takes about 2 minutes.
$1,750 in two payments of $875, one at signing and the second at the start of week 3, or $1,500 paid in full. That is a launch price, for an engagement signed by December 31, 2026; from January 2027 it is $2,400, or $2,200 paid in full. There's a $200 assessment call before we start, credited in full against the program. If either of us ends it by the end of week 2, everything beyond the assessment comes back to you.
By sending this you're agreeing I can email you about it. Nothing else, no list. Your application is deleted within 30 days either way. If you'd rather I delete it sooner, just say so.
